Copies of the following 5 documents must be included in your application to be eligible for our approval process. To download the Code of Conduct and Doctor's Referral forms which you can complete, scan or photograph when complete, click the link next to each name.

Each document below can be submitted in multiple ways. If you are unable to submit the documents online please check next to each field below for the different submission options.

Note: Please ensure each of your files uses a unique name. If you attach multiple files with the same name only the last one will be attached

A $26.25 ($25 + GST) Membership Fee must be paid at the time of application. Please pay $26.25 via e-transfer to membership@tamarackdispensaries.ca after submitting this application.
